equipped Posted November 4, 2011 Share Posted November 4, 2011 My daughter was born in 1996 di pa kami kasal ng mother nya kaya sa surname ng mother ang gamit niya my second son was born 1997 ganun din ang surname pano po ba ang procedure para malipat sa surname ko ang mga anak ko. dati kasi hindi ko may visa na sila isasama ko sa middle east hindi ko naisama kasi ung son ko walang middle name sa birth certificate kaya kahit passport nya wala din. middle name. pano po ba ang procedure wala po kasi dito sa pinas ang mother nila pumirma naman po ako sa affidavit of admision of faternity sa birth certificate nila. thanks in advance.... sweetmia Posted November 21, 2011 Share Posted November 21, 2011 problema po namin yung sa mom ko.. nag try sya apply for passport tapos ayaw sya bigyan kase nakalagay sa birth certificate niya chinese yung father niya.. (1955 po sya pinanganak) di daw sya pede bigyan.. pinapunta kami sa immigration. pag punta dun sabi dapat daw chinese citizen yung mom ko, sa chinese embassy daw kami mag apply for passport.. na-explain na samin yung part ng constitution na nagsasabing chinese talaga siya dapat .. kaso anlaking gastos. baka po may mga alam kayo na ibang way? never na ba talaga sya makakakuha ng philippine passport? Typographic errors refer to misspellings or those which are visible to the eye or obvious to the understanding. Yung mali sa religion, in your case, Islam instead of Christian, ay hindi masasabing "typographical error" lang — kaya kailangan itong dumaan sa husgado; i.e. kailangan niyang maghain ng petition sa korte para ipabago ang birth certificate niya. ano ang steps required: STEP ONE - maghanda ng pera pambayad sa abugado. STEP TWO: kumuha ng abugado... Pero, ayon sa Republic Act 9858, kung ang impediment lamang sa pagpapakasal ng dalawang magulang ay ang kanilang pagiging menor-de-edad nung panahon na yun, maaari na ring maging legitimated ang bata. I'd say kailangang baguhin ang joint affidavit nyo, in such a way, na magiging ganito siya: 3. That when our son/daughter, ______ was conceived and born, the only legal impediment to our marrying each other was the fact that we were both below the age of eighteen at that time; Gumagawa kami ng JOINT AFFIDAVIT OF LEGITIMATION BY SUBSEQUENT MARRIAGE ayon sa documento 3. Cristina, hindi uubra ang affidavit lang. The DFA is mandated to follow what appears in the birth certificate.Dahil Maria Cristina ang nakalagay dun c Certificate of Birth, yun ang masusunod. Kung gusto nyo ng Ma. Cristina, you'll have to change the Certificate of Birth, at yun ay nangangailangan ng Petition sa Civil Registrar - Petition for Change of First Name - may bayad ito tapos ipapa-dyaryo pa ang application, kaya medyo may gastos ito (at least, hindi na sa korte). Magpunta sa Civil Registrar kung saan nakarehistro ang birth certificate ni Tita Cristina at magtanong dun kung ano-ano ang kailangan. Similar case din ung Aunt ko naman sa NSO Birth Certificate nya - Maria Cristinasa other documents school, BIR etc etc - Ma. El Chapo Posted June 28, 2012 Share Posted June 28, 2012 nalilito ako sa DFA, inutusan kase ako ng friend ko magclaim ng papers nya sa DFA yugn may claim stub, then yun pala papa red ribbon pa, sa old DFA daw, pagdating ko dun tinuturo ako sa mga opisina, sila ba talaga may authority na mag red ribbon or pwede rin sa old DFA? lage kase ako tinuturo sa mga office hindi mismo sa DFA kung nageexist pa sila dun. frank martin Posted August 31, 2012 Share Posted August 31, 2012 help naman guys, dinala ko kasi ung kotse ko sa casa sa honda, tapos walang binigay saking checklist sabi balikan ko na lang daw ung kotse. ang pinaayos ko eh ung tensioner bearing + full detailing ng car at ung fender liner. so ok nagawa na daw nila detailing na lang kulang. tapos nung isang araw tumawag sakin di daw gumagana aircon at ang instrument panel ng kotse ko. sabi ko working yan ng dinala ko dyan kaya dapat working yan pag nakuha ko. sa nagyon sabi inaalam pa daw nila kung ano ang nangyari, gusto ko lang itanong in case na wala silang magawa at di nila irepair un ng hindi ako nagbabayad ano ang mga pwede kong gawin? His father was a Chinese Citizen so she carried her father's citizenship and was issued an NSO Birth Certificate showing that she is a Chinese Citizen. On May 8, 1980 Ms. Uy's father was granted Filipino Naturalization and was declared Filipino. Now Ms. Uy wants to indicate in her birth certificate that she is filipino. But since may school days and until now when I sign documents (form 137, SSS, NBI, Marriage contract, etc.) I put Carlota as her name. Me and my siblings grew up calling her that name and even on their marriage contract with my father "Carlota" was used.
