Affleck`` Posted August 14, 2003 Share Posted August 14, 2003 help! my platform at home is WinXP..and few days ago every time surf on the net, esp. when i log in at YM or even just check my Yahoo mail, there's this window that pops up, and says "System Shutdown". it always gives me a minute to save the files im currently working at. then after the countdown, my PC restarts. i think around 5mins after logging on the Internet, dun na sha magpo-prompt. what am i gonna do????????? :cry: Good Thing im running on Win98 hehhehehe safe sa lecheng worm na yan Quote Link to comment

You need to have the following: 1. Certified true copy of the birth certificate of your child2. Affidavit of two (2) disinterested persons attesting to the birth of your child3. Certified true copy of your Marriage Certificate4. Processing fee5. Documentation fee6. Documentary stamps.7. Pang merienda ng gagawa ng bagong Birth Certificate- joke Usually it takes a week or so to process the new Birth Certificate. The requirements sometimes varies from one city or municipality to another. I hope I was able to address your query.Sentinel, in addition to the requirements I stated, you need the following too: 1. Joint Affidavit of Legitimation - this shall be executed by the spouses, if married.2. Certification from the doctor (OB-GYNE) who delivered the child.3. And fill up a verification form from the Office of the Civil Registrar, certifying that the facts of birth appear in the Register of Births from the original. Sana makatulong to. Regards. Art. 364 of the Civil Code:Legitimate and legitimated children SHALL principally use the surname of the father. Art. 376No person can change his name or surname without judicial authority. But there is an exception:Art. 379The employment of pen names or stage names is permitted, provided it is done in good faith and there is no injury to third persons.
