mga doc...EMERGENCY lang

sumasakit ung mga naka circle sa attached pics..parang may bukol sa loob...pero pag huminga ako..hindi naman masakit so thats not the heart...suggestions kung bkt sya maskit...

 

 

kelan sya sumasakit, anu type ng sakit, anu nagpapa agravate ng sakit, sbi mo pag hinga mo di sumsakit? so wen sya sumasakit? describe the mass you felt matigas ba ? o parang my tubig sa loob, movable ba? yun sinasabi mo na area masasakit namumula ba mainit ba pakiramdam?

Mga docs, my son fell down by accident and his bones at his left arm, both the radius and ulna, were broken. It was my ex-wife who brought my son to the hospital and so i didnt see the first xray. They just put my son's arm in a cast and was told to return after 3 weeks. I was the one who brought him back after the 3 weeks had passed and i had him xrayed again before bringing him back to the same hospital. I saw at the xray that both his radius and ulna were really broken and not aligned at all. However, after seeing the xray, the doctor again placed a new cast on my son's arm without even doing anything to align the bones, and we were told to go back again after 3 weeks. I wasnt really convinced about that and so I brought him to the national orthopedic at banawe, qc. We were told that surgery is needed to align the bones since they are really not even near aligning level, and metal screws will be installed. The metal that will be installed is quite expensive. However, i am willing to have it done if it is really necessary. My son is very afraid of the surgery and the pain it will entail afterwards, especially with the metal screws and all. My question is this, is there a possibility that although a surgery is made, that both bones shall be aligned but no metal will be installed? Alignment through hand manipulation and then place his arm in a cast afterwards? I hope to hear your replies as soon as possible as I have already scheduled his surgery for next week. sir, I am not an ortho surgeon but i think that surgery with metal screws and plates and cast is the best way to fix your son's arm. In ENT, we do a lot of maxillofacial reconstruction using titanium plates and screws and to be honest with you, it is very expensive.

Let us wait for the opinion of our ortho member/s.

Fractures of the radius and ulna are almost always treated surgically with metal implants, usually a plate on each bone attached by screws. By this time, the fracture is already about a month post-injury and, if the alignment is not acceptable, unfortunately, the only way to bring it back into alignment is to do what is termed as an open reduction and internal fixation - opening up the fracture site, clearing out the new bone and soft tissue formed over the fracture site until the fracture is visible, place the fracture into its proper alignment, then fix it with the implants. There is no way at this particular point in time after the injury that a good alignment can be achieved by merely manipulating the arm without opening it up and fixing it in a cast. Not in our country nor even in the 1st world. Actually, in the 1st world, they may not even have attempted to cast your son - surgery may have been the automatic option.

sir in my opinion its just a case of strained muscles, or cold nodules....maybe due to your work outs... but its best to consult a sports medicine doctor... or try pt.

I got slightly offended today when I visited a Dermatologist on a health clinic. I was experiencing itchiness for the past few days on my upper back and buttocks so I decided to see a Derma. I didn't know any Derma so I just went to this clinic and was assigned to a young female doctor. When I told her what the problem was she looked at my upper back. However, I mentioned that the itching and rashes were worse on my buttocks so she decided to take a look at it also. However, before doing so she called a male nurse from outside to accompany her while checking it out. I felt slightly offended cause she felt that she needed somebody to be with her as if I am going to harass her. Hindi naman ako manyak (specially kung professionals yung kaharap ko). And there are around 8 more patients on the hallway just outside the door so I would not be able to do anything even if I wanted to.

 

My question is - is it ethical for a female doctor to call a nurse just to "guard" her when inspecting a male patient's buttocks? Considering she is a doctor, she should already be used to seeing butts.

sir, this has become more or less standard procedure in recent years and let us be clear on this---it had a lot more to do with protecting yourself, (the patient) from her (the doctor) than the other way around. we do this just so both parties involved attain a certain level of comfort while the exam is performed.

in your case, I apologize on behalf of my colleague if it offended you but rest assured nothing inappropriate was done at all.

Thank you for your response. It gives me some relief now that you say that this has become a standard practice. I did not know. However, a female doctor did the Annual Physical Exam in our company and some of the male staff were assigned to her without anybody else during the exam.

 

I understand the comfort level part of it although a patient might feel more embarassed with more people instead of just the doctor doing the exam. However, I don't understand how such arrangement could be protecting me (patient) instead of her (doctor). Maybe you can enlighten me further. TIA.

if say, a woman came to see a male doctor for say, vaginal spotting or bloody stools, it would be incumbent on the part of the physician to have a female nurse or co-worker around while performing the pelvic or rectal exam to reassure the patient that the exam is being done appropriately by the doctor and not to misinterpret certain actions or maneuvers as sexual advances.

True. In the medico-legal sense also, it is also protection for the doctor. If, for whatever reason, the patient feels that he or she was physically violated and decides to press charges against the physician, the doctor has a witness of the same sex as the patient who can describe how the examination was done - whether it was appropriate or not, thus saving physician from a punishment which he does not deserve, assuming the procedure or examination he did was appropriate and within acceptable standards.

Hopefully there are guys (or hopefully those from the medical field who know) who have had their tonsils removed here. I am bothered with tonsillitis at least 4 times a year and is contemplating about having it removed.

 

Does anyone know how much would this cost me? Please share. I have heard about laser ablation - an out patient kind of procedure wherein the tonsils would basically get vaporized by laser. INot sure is this procedure already exists here, and for what costs?

 

Inputs would be greatly appreciated. Thank you.

hello good day.. im not an ophthalmologist but just like to share my opinion...regarding the retinal swelling..since your dad is currently seeing an ophthalmologist it is best to have regular follow ups regarding this case...investigation will be necessay in such that prognosis will be gauged and proper treatment options done..as for the eyedrops or steroids to reduce the inflammation...i think what is feared on retinal inflammation is its possible detachment which will be detrimental to the vision of the individual...in such..surgery is warranted to reattach the retina...as for the cataract..it is caused by several factors such as old age..trauma..or medications...in cataract..the lens that is normally transparent becomes opaque due to deposition of proteins...causing failure of the lens to direct the light to the retina...symptoms include blurring of vision/cloudiness, dizziness, photophobia/glares and decreasing visual acuity...as for the treatment options....surgical management such as phacoemulsification and implantation of intraocular lens is commonly done wherein the diseased lens is removed and artificial lens is placed...this is a short process often lasting for less than an hour...in some cases if the cataract is immature..the specialist may advise the patient to have regular follow ups until the cataract is viable for surgical removal..

in addition to whiteshadow's reply, ask your ophthalmologist kung hindi glaucoma ang cause ng redness and pressure in your dad's eye, that condition kasi can lead to permanent blindness kung di naagapan. the cataract could be contributing din to increased pressure in his eye

Your ophthalmologist is in a better position to answer your querries.

I am just concerned about the explanation that the retina is "namamaga" causing the pain. I am not an ophthalmologist but as far as I can remember, most, if not all, retinal diseases usually do not present as eye pain.

 

A second opinion wont hurt.

Ngayon ko lang nabasa itong concern mo. Hirap lang sa mga online consultations, walang physical exam involved kaya ang hirap din mag bigay ng tamang sagot. But I do understand your concern.

 

Majority of retinal conditions do not involve pain. However, those that do could be due to a number of things. Some pretty serious. Considering that you have already approached an ophthalmologist for help, the smart move would be to heed his advise. If doubt still clouds your mind, siguro you can go to another ophtha for a second opinion. I'd suggest you go to a retina specialist. I used to be a practicing ophthalmologist and whenever I have made an initial diagnosis, I often refer my retina cases to retina specialists.

 

Regarding sa gamot, i hope you understand the limitations this thread affords both you and the doctors here. Mahirap magbigay ng reseta na haw-siao lang ang pinagbabasihan.

 

Yung cataract naman eh walang gamot. Yung ibang binibigay, pang pa-tagal lang ng panghinog ng katarata. Dati kasi, kailangan hinog na ang katarata bago ito maoperahan. But with today's technology, hindi na kailangan maghintay pa. Just ready your father for a cataract extraction surgery which is fairly simple and common. Doon din ang bagsak kasi niyan.

 

I wouldn't suggest you be bothered by this in light of another eye problem though. Benign condition naman ang cataracts. Unless the pain is actually due to a secondary glaucoma which is cataract-induced. But I'm sure na rule-out na ito ng ophtha mo.

Hello to all the medical peeps out there... I just want to ask, how ill you know if the woman is really pregnant?

I mean my girlfriend is so paranoid she might be pregnant. she is kinda delayed for only 30days palang.

I didnt use any condoms when we last did it but i withdraw it naman... and her menstrual cycle is irregular even before we met. what should i do? help naman po...

the best thing to do---to alleviate all doubt (by MTC experience )---is to consult your friendly neighborhood OB-GYN.

 

I'd advise against procuring the commercially available pregnancy test kit because:

 

1) while it may conclusively tell you whether or not your gf is pregnant it doesn't answer all your other questions i.e. if she tested negative, why isn't she having her menses? a consult will be necessary since she has been irregular as you said, for like forever. while this may likely be benign in the big picture (secondary to stress, diet etc.), it would be judicious to rule out any underlying pathology (PCOS, uterine abnormalities, etc.).

 

2) upon consult, an OB-GYN will order the test to be done at her clinic anyway (para maiwasan ang ahem, pandodoktor).

 

and oh, lastly---the withdrawal method doesn't really work that well dude.

so unless you really plan to have a little one running around soon, rubber's your new BFF.

i think a pregnancy test would be a nice initial remedy just to dispell the patient's anxiety. now if questions still arise after the results, then jecq2 can probably be a gentleman and accompany his gf to an ob-gyn.

 

jecq2, buti nalang naisingit mo na irregular ang cycle ng gf mo. natawa naman ako ng binanggit mong there's nothing to worry about kasi 1 month pa lang siyang delayed. kasi nga sa babaeng regular ang cycle, matagal na ang 1 month.

let's just say that there have been a few interesting posts regarding this matter in the not-so-distant past bro (that's why I indicated "by MTC experience").

in the end, if she does test positive, well, there you have it---they're gonna end up with a consult anyway.
